Booking
The first thing you need to do is book your appointment! You can fill out our Contact form below or you can call our office at 801-685-3398. We will confirm your appointment and you receive a reminder call, text or email (depending on the information you gave us) one week, and then one day before your appointment. Don’t forget if things change and you need to reschedule, we are happy to do so, just please let us know!
Arriving at the Clinic
When you arrive at 22 S 900 E, our signs say Ear, Nose, & Throat Center. Eyelid Center of Utah is inside! Dr. Eftekhari works closely with this premiere group of ENT specialists in Utah. Our front desk staff will greet you and help you get checked in – depending on whether you are a new patient or a returning patient, there may be some paperwork for you to do. If you have an insurance card, don’t forget to bring that along with your ID to get checked in. Then, you can have a seat in our waiting room – have a look around at our informational brochures!
Your Appointment
You won’t be waiting for long! One of our medical assistants will come to collect you when the appointment room is ready for you. For medical visits, we need some vitals from you, and we will take those readings as well as get a standard set of facial photos that are securely saved into our electronic medical record. For cosmetic visits – no extra hassle, just fill out our simple questionnaire while you wait! Then, Dr. Eftekhari will come in to see you and discuss the reason for your visit. Depending on your condition, Dr. Eftekhari may do other testing as well.
After the Appointment
Dr. Eftekhari will review your specific concerns with you and recommend the best course of action going forward. This could be:
- A simple follow up – you can schedule this with his medical assistant or at the front desk
- An In-Office procedure – BoTox or filler injections, a biopsy, or minor procedure
- A scheduled surgery
In the case of a scheduled surgery, all we need is a consent form signed and you will be going home with our surgery folder, which includes information about surgery, including pre- and post-operative instructions. As a courtesy, we will check your benefits and contact your insurance company for any medical procedures. For cosmetic procedures, you will get a written cosmetic quote at the time of your visit. We will check your benefits to determine which facility will be in-network for your insurance or have the lowest fees, and you will be contacted by us or our Surgery Center schedulers within 2 business days.
If you have any questions for us, you may call in advance or bring a list in with you to the appointment. We will do our best to answer in as much depth as possible, and give you further resources if we can! If you need special assistance (e.g. translation), we can arrange this as well, just be sure to let us know in advance! We hope to see you soon here at the Eyelid Center of Utah!
